If you want to join PATHFINDER, please follow the following guidelines that we have designed for you to assist you with your application
 
How to Become a PATHFINDER member?
 
Welcome to the Medical Scheme which specialises in making Healthcare affordable and accessible without sacrificing the essentials of Healthcare cover.
 
Please follow the following easy steps to become a valued PATHFINDER member:
 
1. You need to complete a PATHFINDER Membership application form. Please note that you need to indicate your chosen Contracted GP for the PATHFINDER Trail option. You can obtain a list of all Pathfinder networks by phoning 0861 07 08 09 or you can visit our web at www.pathfinderhealth.co.za.
   
  You also need to indicate your date of joining on the application form. Please note that contributions are collected monthly in advance on the 1st working day of every month.
   
  The debit order close off date for new applications is the 20th of every month, however you can pay in your first contribution should you still wish to have a join date of the 1st of the following month.
   
  Please contact a skilled PATHFINDER consultant on 0861 07 08 09 if you need advice on determining your joining date.
   
  Once you have fully completed your application form:
   
2. You can fax your application form to 012-6639981 with the following documents:
   
 
· Copy of ID document.
   
· Proof of Income for Trail option.
   
· Membership certificate of previous medical aid.
   
3. Once PATHFINDER has received your application form we will process the documentation within 2 working days. Please note depending on previous medical aid membership and your health status, we may impose certain restrictions for e.g. a 3 Month General waiting period or 12 month specific condition waiting period.
   
4. In the event that restrictions are imposed, PATHFINDER will forward you a letter indicating the restrictions, it is up to you to decide to accept the terms of acceptance or not. Should you agree with the new conditions please sign and fax the letter to 012-6639981. Please note that your membership will not be activated without your consent, so in the event that you choose not to become a member, your application will not be activated.
   
5. Once we have received your acceptance your membership will be activated in accordance with the requested inception date indicated on your application form. Should you however be accepted without any conditions the application will be activated automatically without any further advice from you.
   
6. Your membership card will be posted to you within 5 working days of activation of membership.
   
7. If you are currently a member on a Medical Scheme please ensure that you cancel your medical scheme once accepted by PATHFINDER and that your cancellation date of your current scheme is linked to the inception of PATHFINDER. Please remember that your current scheme probably requires at least 30 days notice. Please note by law you may not be on two medical schemes at any given time.
